This form maps every applicant-facing item from the four three-page forms published on the legacy website and automatically matches the instrumental or writing version. Admissions reviews the complete submitted record in the YCMA admissions system.
The form retains every applicant field, checklist item, declaration, and office-review item from the source forms, but it does not collect passports, financial records, or supporting documents. Admissions provides a secure submission route when required.
